STANDARD OFFICE POLICIES & FEE SCHEDULE

Neurowellness Office Policies

Payment: NeuroWellness requires payment for services at the time they are rendered. Payment may be made by cash, personal check or credit card (MasterCard or Visa). A fee of $25.00 will be charged for returned checks.

Appointment Cancellation: NeuroWellness requires that (out of courtesy to others) cancellations for scheduled appointments be received 24 hours in advance during regular office hours (Monday through Thursday 10:00am to 5:00pm). Missed or canceled appointments that do not follow this policy will be charged an appointment fee. This fee can equal but will not exceed the fee for the time originally scheduled. To reserve your appointment, a credit card is required.

Insurance Billing : We do not bill insurance companies. On request, for Neurotransmitters therapy we will provide receipts that may be submitted to insurance carriers for reimbursement. Clients/Responsible Parties are responsible for all charges whether or not they are covered by your insurance. In the event an insurance company requires the entire client's file, there will be a fee to prepare and send it.

Explanation of Fees: The fees charged by NeuroWellness are based on the amount of time scheduled for each person (in the office or on the phone). The minimum amount of time scheduled/charged is for a follow-up session (20 minutes in length). If additional time beyond the scheduled time is taken, there will be a charge for the amount of time used. If you have a simple question between appointments please feel free to call and they will be addressed at no additional charge. In the case of question that requires a complex answer please schedule an appointment so your question or concern can be addressed properly. The cost of supplements is in addition to the consultation fee. In addition, there is a small administrative fee for time taken to write reports or correspondence on a client’s behalf. Important: Above Pricing does not include the cost of Supplements.
Supplementation regimens are recommended based on the individual's unique
nutritional situation and therefore must be separate from the standard consultation.
